Counter-Notification

If you believe your content was removed in error or misidentified, you may submit a counter-notification containing the following:

Your name, address, telephone number, and email address;
Identification of the material that has been removed and its location before removal;
A statement under penalty of perjury that you have a good faith belief the content was removed or disabled as a result of mistake or misidentification;
Your consent to the jurisdiction of a federal court in your location (or the owner's location if outside the U.S.);
Your physical or electronic signature.
